In FY20/21 our understanding of the impact of COVID both on individuals, the system and society as a whole was limited and still forming. In early FY21/22, we established the Live Well Programme, which looked at providing support to the people who were affected by the impact of the COVID-19 pandemic, as identified by the Welsh Government. These populations groups consist of those recovering from COVID, those experiencing delays in their care, those who have been avoiding accessing services, and those suffering from the effects of isolation.
The overall aim of the Cardiff and Vale UHB Rehabilitation Programme is to support people to access rehabilitation services closer to home that support them to recover and make good choices to live well.
The programme aims to provide an Allied Health Professional-led response to the challenge and begin to shift the model of healthcare for those with today’s long-term health conditions
